About the Role

The Ambulatory Support Assistant Support the Ambulatory Care Management Department and primary care practices with a variety of tasks. Your role involves care coordination, scheduling appointments, processing referrals, completing forms, and connecting patients to resources. You will collaborate with the care management team, primary care practices, and patients/families to deliver quality, efficient, and cost-effective healthcare services.

Qualifications:

  • High school diploma or GED required.
  • Minimum of 2 years' experience in healthcare or human services.
  • Basic knowledge of medical terminology.
  • Experience with adult and pediatric patients in primary or post-acute care settings preferred.
  • CHW certification, CNA, or a degree in healthcare or human services preferred.
  • Proficient in word processing, spreadsheets, presentations, databases, and other relevant software.

Essential Duties:

  • Scheduling appointments and coordinating follow-up care.
  • Supporting care coordination by connecting patients with appropriate resources and care team members.
  • Assisting patients with completing forms and applications.
  • Collaborating with healthcare team members, patients, and families to optimize patient health status.
  • Implementing targeted interventions to increase patient self-management and prevent hospital readmissions.
  • Maintaining required documentation and adhering to departmental guidelines and protocols.
  • Supporting quality improvement projects and fostering a team-based approach to patient care.
