The advanced clinical pharmacist demonstrates proficiency in all pharmacist functions to include: Attending and actively participating in daily multidisciplinary and bedside rounds, completing drug utilization review to promote rational drug therapy, leading and developing clinical services, mentoring and educating staff to ensure accurate and appropriate documentation of clinical activities. Conducts drug regimen reviews and responds to specific polypharmacy, pharmacokinetic and medication/MAR reconciliation consults. Recommends and monitors drug therapy as assigned by the medical staff. Utilizes expertise to train, mentor and supervise PharmD candidates, pharmacy residents, and technicians. Train and participate in facility wide committees and training programs.

In this role, the Clinical Pharmacist will:

  • Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.

  • Sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection

  • Attend and actively participate in daily multidisciplinary and bedside rounds

  • Support the hospital formulary decision process by working with medical staff to promote rational drug therapy and make improvements identified by drug utilization evaluations

  • Provide accurate and timely medication information to healthcare staff and patients

  • Actively participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.

  • Participate in the antimicrobial management and other clinical programs

  • Document all clinical activities and interventions accurately and completely. Responsible for mentoring and education as well as ongoing analysis of interventions to insure accurate and appropriate documentation and provide summary reports (if assigned)

  • Support and participate in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.

Requirements:

  • Education: ONE of the following:

A. Doctor of Pharmacy (PharmD)

B. Bachelor of Science in Pharmacy

  • Experience: One of the following is required:

  • Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience required; preferably within area of focus Completion of hospital PGY1/ pharmacy general practice residency or fellowship BCPS certification or other Board of Pharmacy Specialty (BPS) certification 3. Employee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable).

Minimum License, Certificates, or Registrations Required: A. B. ONE of the following prior to the first day of employment:

  1. Current Registered Pharmacist licensed by the Texas State Board of Pharmacy

  2. Licensed pharmacist in another state: Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy ACLS/PALS and NALS required for critical care/ED position dependent on population served.
