Care Management Support Coordinator II BCforward is seeking a highly motivated Care Management Support Coordinator II role, location Remote EST or CST Job Title: Care Management Support Coordinator Location: Remote, EST or CST Target Start Date: March 24, 2025 Pay Rate: $19 per hour Assignment Duration: 4 Months (possibility to extend or convert) Job Description: Care Management Support Coordinator Position Purpose: The Care Management Support Coordinator is responsible for supporting administrative care management activities, which include performing outreach, answering inbound calls, and scheduling services. The role serves as a point of contact for members, providers, and staff to resolve issues and maintain accurate member records in accordance with state and regulatory guidelines. Education/Experience: * High School diploma or GED required. * 1 - 2 years of related experience. * Experience in customer service or healthcare is preferred. Key Responsibilities: * Perform outreach to members via phone to discuss care plan next steps, community or health plan resources, and any questions or concerns related to scheduling. Provide ongoing education for both the member and provider throughout care/service. * Assist members in connecting with other health plan and community resources to ensure they receive high-quality customer care. * May apply working knowledge of assigned health plan(s) activities and resources. * Serve as front-line support on various member and provider inquiries, requests, or concerns, including explaining care plan procedures and protocols. * Support member onboarding and administrative duties, including sending out welcome letters, program educational materials, and other correspondence to assist in building a successful member/provider relationship. * Document and maintain non-clinical member records in accordance with state and regulatory requirements. * Make referrals to address Social Determinants of Health (SDOH) needs, leveraging knowledge of local benefits and resources. * Perform other duties as assigned. * Ensure compliance with all policies and standards. Work Environment: * Work is remote, within the EST or CST time zone. * Daily schedule is from 9:00 AM - 6:00 PM EST (flexible). * No overtime expected. * Primarily individual work, offering case management and connecting with Medicare members via phone. Key Performance Indicators: * Quality phone service. * Ability to work effectively in a team environment and assist others when needed. * Metric-based performance (engagement, declines, and UTR).
