One Health Drop-In-Center Peer Support Specialist in Harlem, Montana

POSITION SUMMARY: Functions as a core member of the Drop In Center outreach team that involves the Drop In Center Manager, RCAT Team, and other community partners. The peer support specialist is responsible for providing and coordinating warm handoffs and mentoring to patients or potential patient families who are impacted by SUD (substance use disorder) and in need of behavioral health services and/or resource support. The Peer Support Specialist (PSS) also provides experience-based input to guide the implementation of the strategic plan for serving local families according to the community’s unique needs.

COMPENSATION: $18.31-$18.91/hr DOE

HOURS: Monday-Friday

MINIMUM JOB QUALIFICATIONS:

Education/Certificates/Licenses:

Required

  • State certification for Peer Supporters (required to complete Peer Support 101 and begin certification filing process within 30 days of hire)

  • BLS Certification upon hire

  • Valid driver’s license

  • Master Narcan Trainer Certification (required to complete training within 60 days of hire)

  • Certified Trainer for Fatherhood is Sacred/Motherhood is Sacred (required to complete within 90 days of hire)

    Experience:

    Required

  • Lived experience with SUD

  • Minimum two (2) years in recovery

    ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

  • Maintains stable sobriety and be willing to self-identify publicly as being in recovery from a mental health, substance use or addiction diagnosis.

  • Obtains/maintains state certification for Peer Supporters.

  • Understands the concept of recovery as it relates to behavioral health; peer support/recovery coaching; wellness and recovery planning.

  • Facilitates patient engagement and follow-up care.

  • Documents in-person and telephone encounters to report to the Drop In Manager.

  • Facilitates referrals for clinically indicated services and resource support outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care).

  • Participates in multi-disciplinary, team-based care of patients/clients.

  • Participates in training, reporting, and other requirements of various grant supported programs implemented by One Health and partners.

  • Teaches at least one section of the Fatherhood is Sacred per year.

  • Engages fathers and male partners and family members in healthy relationship education (manhood mentoring).

  • Provides peer support for male participants at the drop in center and in the community.

  • Offers Narcan training as often as needed for community groups and citizens.

  • Hosts at least one weekly recovery support group.

  • Hosts at least one weekly recovery fitness group.

  • Adheres to all One Health Policies and Procedures.

  • Host Narcan training for community groups and individuals.

  • Participates in performance optimization and risk management activities as required.

  • Cooperates and abides by all elements of One Health’s Compliance Program.

  • Provides exceptional customer service and models the mission of One Health and of the Drop In Center when interacting with internal and external stakeholders promoting a positive image for the health center.
