LifePoint Health Social Worker (MSW) FT Days in Gallatin, Tennessee
Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Reports to: Director - Case Management
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Drag Edit Delete
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. Drag Edit Delete
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Drag Edit Delete
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Drag Edit Delete
Supervises and/or trains new staff, students, and interns. Drag Edit Delete
Performs Medicaid screenings as indicated for NHP. Drag Edit Delete
Assists patients with Safety Net applications when needed.
Minimum Education Master's degree in Social Work - Required Drag Edit Delete
Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Drag Edit Delete
Required Skills Certifications: BLS certification from the American Heart Association (AHA) or American Red Cross (ARC) - Required within 90 days of employment, if not current upon hire Drag Edit Delete
Required Skills Licenses: State of Tennessee Licensed Master Social Worker - Preferred Drag Edit Delete
Minimum Work Experience Three years of clinical experience in as a Social Worker in acute care setting - Preferred
