Act as patient liaison to coordinate follow-up care, infusion coordination, medication management, and clinical monitoring. Provide clear, constant communication regarding patient status changes with team members with daily updates on complex care patients and delegating as needed. Identifying resources for patients in the community that can aid in the physical, social, spiritual, and financial burdens of chronic medical conditions. Store information and provide information to team members. Act as clinical resource for nurses, LPNs, MAs, and PA coordinators Provide education to new or ongoing patients on the possible adverse effects that may occur with medication therapy, required monitoring for medication management, infusion experience, possible infusion related reactions, and symptom management relating to medical diagnosis. Initiating and tracking prior authorizations for medications as needed by providers. Must be in frequent communication with patients regarding status of authorization, as well as clear documentation of status changes in ECW Create a calm environment that ensures patient comfort and trust throughout the duration of their care Meticulous documentation skills Develop care plans with providers and other members of the healthcare team. In the event of staffing shortage in infusion suite, provide nursing care coverage. This includes intaking patients, establishing IV access, mixing medications, administering medications, managing infusions/reactions, discharging IV access, and providing discharge instructions per protocol.
