Salud Family Health Centers Women s Wellness Connection Patient Navigator in Fort Lupton, Colorado

As a grantee of the Colorado Department of Public Health and Environment (CDPHE), Salud is able to offer the Women s Wellness Connection (WWC) Program to eligible patients for cancer prevention and early detection. The WWC Patient Navigator functions as a member of the patient care team performing patient navigation for both insured and uninsured women through the Health Navigation and Clinical Services component of the WWC Program. The WWC Patient Navigator approaches his/her tasks in a team-based care fashion and provides support and guidance to patients throughout the breast and/or cervical cancer screening process.
We offer: Starting Wage - $18.93 to $21.77 per hour Monday to Friday 8 am to 5 pm or 7 am to 4 pm

Qualifications Education: High school diploma or GED, required
