As a reputable provider of exceptional care and services for adults and seniors, we are seeking a dedicated professional to join our subacute care team, ensure outstanding patient experiences, and foster a vibrant and engaging environment.

Aurora on France TCU is a fast-paced, high-quality care facility conveniently located in Edina, MN, and connected to M Health Fairview Southdale Hospital. Our dedicated and talented team works collaboratively to ensure patients meet their goals of care and return to their homes in the community. As the Household Coordinator, you will have the opportunity to be a part of a vibrant team, empower patients, and create lasting impacts while you work to achieve the goals of initiating positive transitions into the facility, advocating for the needs of our patients, and facilitating a smooth and safe discharge back into the community.

This Household Coordinator position will be tasked to provide efficient and customer friendly admissions/discharges for our TCU patients, with the support from our social services department. Schedule includes

80 hours every two weeks

Day shift, no weekends

  • Positively support the culture of the household (TCU unit) by leading with a solution focused mindset

  • Create an exceptional experience that prioritizes safety and service to improve the experience, health, and wellbeing of everyone.

  • Facilitate a smooth admission into the facility through the completion of intake assessments and admission paperwork

  • Ensure the psychosocial needs of the patients are met

  • Facilitate positive and safe discharges and ensure patients have the services, healthcare, and equipment they need to thrive in the community they are returning to.

  • Serve as the patient’s advocate and oversee patient quality of life in the facility.

  • Identify, investigate, and resolve complaints and grievances on the unit.

• Completed or working towards an Associate’s or Bachelor’s degree in Healthcare Administration, Social Work, or a related field.

• Exceptional interpersonal and communication skills, with the ability to build positive relationships with patients, families, and staff.

• Dedication to patient-centered care and a genuine passion for serving seniors with dignity and respect.
