Dexter Health Care is seeking a Charge RN that is interested in becoming an MDS Coordinator. Learn while you earn at a top-level caring facility.

The Charge RN provides leadership, clinical oversight, direction, monitoring, and training for all nursing and nursing related operations for the facility during your scheduled shift including: clinical management of residents, customer satisfaction, regulatory compliance with state and federal regulations, quality improvement and outcomes, policy and procedure enforcement.

Education & Licensing Requirements:

  • State of Maine Nursing license as a Registered Nurse. (required)

    Essential Job Functions

  • Ensure that nursing practice complies with regulatory and legislative requirements.

  • Ensure sufficient numbers of licensed and certified staff are available each shift to meet regulatory requirements and provide appropriate care to residents.

  • Complete routine clinical rounds each day to ensure all staff are performing work assignments in accordance with acceptable standards.

  • Promote, establish, and maintain customer relationships with residents, families, staff, community, and other professionals.

  • Review complaints and concerns made by families, residents and staff, communicate these concerns including follow up with the managers, or Director of Nursing, or Administrator as appropriate.

  • Provide support for staff competence with electronic charting system (ECS).

  • Make written and verbal reports/recommendations to the Managers and Director of Nursing as necessary concerning the operation of the nursing department.

  • Assist and implement appropriate plans of correction for safety concerns identified. Ensure assessments, evaluations, and charting is completed according to policy.

  • Actively participate in Quality Assurance and Performance Improvement (QAPI) program as directed by the managers or Director of Nursing.

  • Participate in routine and daily care of residents to include but not limited to assistance with ADLs, meals, eating, functional maintenance plans/nursing restorative plans, and activities.

  • Complete and deliver assigned staff performance evaluations as directed.

    What you will learn:

    MDS Coordinator General Purpose:

    As part of a high-functioning leadership and care team, conduct and coordinate the development and completion of the resident assessment in accordance with the rules and regulations of State and federal government and the policies and goals of the facility. Reports to the Director of Nursing.

  • Assess/Oversee assessment of all residents through the RAI process and ensure coding accuracy, capturing needs and level of assistance, in accordance with current rules, regulations, and guidelines

  • Review all care plans on a regular basis for accuracy, appropriateness, and case mix validity

  • Oversee and/or complete care plans for all resident’s process according to standard of nursing practice, including nursing restorative plans

  • Establish documentation approaches and instruct nursing and ancillary staff to document properly

  • Communicate MDS due date so that interdisciplinary team members complete their portions appropriately and on time.

  • Instruct all disciplines in proper procedures, completion of MDS items, and CAAs, Care Mix payment and Quality Indicators.

  • Must have ability/competency, and willingness to work as a direct care provider when necessary.
