Job Summary and Qualifications

We are seeking an Oncology BMT Social Worker III for our facility to ensure that we continue to provide all patients with high quality, efficient care. Here is some of what you can expect from being a Social Worker III with us:

  • Work with inpatient and outpatient multi-disciplinary oncology team in order to identify and meet needs of oncology patients as they progress from diagnosis through survivorship

  • Thoroughly assess the psychological, social, cultural and or financial needs that may impact patients going through transplant.

  • Utilize counseling and clinical knowledge in order to make recommendations and provide appropriate support for patients and their families through the transplant process

  • Demonstrate strong verbal/written communication with providers, patients, families and other members of the care team as well as the ability to resolve conflicts and creatively solve problems related to psychosocial needs.

  • Conduct discharge planning assessments and make recommendations and referrals to post-acute facilities as necessary in order to achieve optimal patient outcomes.

  • Provide counseling in individual and support group settings for patients/caregivers during and after treatment

  • Assist patients with applications/documentation for work leave of absences, grants, and/or referrals for assistance applying for SSDI, Medicaid or COBRA

  • Identify appropriate time for end of life discussions and involvement of palliative and hospice care resources

  • Engage in ongoing education related to BMT/Oncology Social Work and pursue professional certifications

What qualifications you will need:

  • Master of Social Work (MSW) from an accredited school of social work

  • Licensed Clinical Social Worker (LCSW) in the State of Colorado.

  • 3-5 years recent clinical social work experience in a healthcare setting. Experience working with oncology patients preferred.
