Job Summary

Under general supervision, assists in developing and implementing comprehensive diabetes patient education program. In collaboration with unit based leadership, the CNE assists with patient and family education and staff training. Assists with ongoing program assessments and assures patients and families receive education that is evidence based and meets community and national standards. Additionally, the CNE participates in the coordination and facilitation of interdisciplinary patient centered diabetes care across the continuum.

Essential Functions:

General Operations: * Serves as a resource for patient-directed Diabetes Self-Management Education, Support and Training. Utilizes knowledge and skills to support informed decision making, self-care behaviors, problem solving, and collaboration with health care team to improve clinical outcomes, health status and quality of life. * Supports the staff providing diabetes education as a coach, mentor and clinical expert. * Collaborates with the interdisciplinary team to assess blood glucose monitoring and pattern management. Collaborates in the use of diabetes medication therapies to achieve and maintain optimal glycemic control. * Assists the interdisciplinary care team to assess patient related social, economic, and cultural aspects (including health literacy, barriers to learning, and readiness to learn) in applying/adhering to diabetes self -manage * Provides individual patient education and diabetes management (including, but not limited to medication titration per physician-approved protocol) services in ambulatory clinic setting. * Acts as a professional educational resource to the interdisciplinary patient care team. * Demonstrates an understanding of assessment findings as they relate to disease processes and continuum of care; relates findings to multidisciplinary team. * Evaluates practice in relation to professional standards, statutes, regulations, hospital and departmental policies, order sets, protocols, and standards of care. * Enhances professional growth and development through participation in educational programs, reviewing current literature and attending inservice/staff meetings. * Documentation: Documents the outcomes of the educational process Adheres to the Denver Health principles of Informing Clinical Practice, Improving Population Health, and Personalizing Care. Performs documentation audits. (45%) Collaboration: * Works with Management Team to meet diabetes program goals. * Collaborates with leadership to improve quality and efficiency throughout the continuum of clinical care. * Assists with research into professional and community practice standards, technology, and clinical innovations for improved service within assigned care area. * Maintains close, effective and accurate communications with the interdisciplinary team. Accumulates and articulates information needed. * Coordinates care with multidisciplinary team. * Interacts with colleagues and the public in a manner consistent with the standards set forth in "Excellence of Service" and the mission of Denver Health. * Participates in the peer review process. (15%) Data Analysis: * Systematically analyzes and interprets the needs assessment data using an expert knowledge base to formulate a diabetes education/competency program plan that is based on desired outcomes. * Integrates research behaviors into clinical practice by demonstrating awareness of research findings, identifying areas of practice that can be improved trough research utilizati n and assisting in research. * Collects data to promote improvement. * Demonstrates an understanding of assessment findings as they relate to specific patient populations and contractual obligations. (15%) Implementation/Outcomes: * Guides students and practicing nurses, identifies their learning needs, strengths and limitations, and select learning opportunities that will build on strengths and overcome limitations. * Designs, implements, and evaluates the diabetes program that meets the learning and competency development needs of patients/family and clinical staff (10%) Safety/Quality: * Practices sound safety and infection control practices according to agency policies and procedures; Identifies safety and infection control issues. * Evaluates quality, effectiveness and efficiency of nursing care provided. * Performs qualify control on appropriate systems and equipment. * Promotes quality care by redefining priorities for care based on evaluation
