Work hours: PRN (as needed) - covering days (7a-7p)

Here’s What You Need
Diploma in Nursing required or
Associate degree from an accredited Nursing program
BSN preferred
3 years RN experience required
Certification in specialty fields such as acute care, oncology, or geriatrics is desirable.
Critical Care or Med/surg experience preferred
RN - Registered Nurse upon hire required
CPR within 30 days of hire (maintained every 2 years)
What You Will Do
The Virtual Care RN is responsible for providing and coordinating high-quality patient care to various patient populations within the framework of the Advanced Hospital at Home mode. The nursing care is provided from a centralized location, supported by technology and service provider network. Assesses the patient
The Virtual Care RN is responsible for providing high-quality patient care to various patient populations within the framework of the Advanced Care at Home model and includes the following key elements:
Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home
Navigating patients and families across the continuum of the Advanced Care at Home phases of care
Monitoring and responding to biometric data
Conducting home and audio/visual patient care visits as directed by the care team and/or guidelines
Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach
Leveraging the resources and expertise of the Advanced Care at Home virtual care team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical evidence as the foundation
Adhering to evidence based practice and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care
Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts
Quality and Safety Responsibilities:
Ensure compliance with documentation and reporting requirements for Sentinel Events and Serious Incidents
Participate in quality and safety investigations and root cause analyses
Take appropriate action to ensure patient safety as a result of any investigation; and
Serve as a resource in quality improvement processes and technique
