Job Description

Position Summary: A LINK Case Manager’s s responsible for providing treatment to the child and their family/caregivers to address the clinical needs of the child and the complex needs of the family unit. The LINK Case Manager will provide crisis intervention and stabilization with the child/youth and family, and case management services such as referrals to other services and ensuring the structural home environment is safe.

  • Responsible for three to four assigned cases and will work with the identified child(ren) and their families for a period of 4 to 6 weeks.

  • Flexibility with his/her schedule in order to meet the needs of the families served.

  • Available to all families in program 24 hours a day, 7 days a week, for the purpose of crisis intervention and support, on a rotating basis.

  • Provide back-up case coverage to a fellow team member when needed.

  • Work intensely with a family, in their home and community, as frequently as needed. As the caseload is kept small, a Case Manager will visit the family as many times as necessary during the course of the week, with a minimum of three visits weekly, and daily phone contact.

  • Provide education to families regarding children’s mental health, teach behavior modification techniques, parenting skills, and provide parents with crisis stabilization strategies.

  • Collaborate with community treatment providers, schools, other organizations, etc. to ensure continuity of care for youth and families served.

  • Complete all necessary paperwork related to his/her cases, including but not limited to, home safety assessment, intake face sheet, intake evaluation and psychosocial history, admission/ discharge CAIRS, child strengths/ needs, safety alert plan, goal plan, goal plan fourth week review, goal plan termination review, progress notes, authorizations of release of information, consent forms, continuity of care document, discharge/ aftercare plan, and termination summary. A Case Manager will complete all documents thoroughly and submit them to his/her supervisor by the required due dates. A Case Manager will be responsible for maintaining case records consistently.

  • Provide families with appropriate linkages and assist with relevant community resource identification.

  • Meet with the Program Supervisor weekly for clinical supervision to review cases. A Case Manager will prepare for supervision and display a readiness to discuss case issues, including interventions and linkages.

  • Prepare and meet with Program Director, and LINK team, every morning, to discuss updates and status of their cases.

  • Participate in weekly team meetings, and act as a team member by providing mutual support, brainstorming of ideas, constructive feedback, etc.

Qualifications

  • Master’s degree or Licensed mental health provider preferred.

  • Bachelor’s degree in human services with two years of experience or Master’s degree with one year experience.

  • Must have valid and clean driver’s license.
