Position Summary The Community Health Worker (CHW) position works directly people at risk for HIV, HCV, STIs and TB. This position will offer rapid HIV, Hepatitis C and Syphilis testing as well as provide syringe exchange services. The CHW will work directly with people who use drugs identified at syringe exchange, street outreach and through referrals from community partners. The CHW will play a critical role in utilizing harm reduction strategies and motivational interviewing to empower clients to take steps to reduce their risk for overdose, improve their health, and access treatment. This position will work in collaboration with a network of community providers to mitigate the negative impact of substance abuse on PWUD via evidence-based services and resources that reduce risks for HIV/HCV and opioid overdose, promote recovery, and improve general health outcomes. While not all clients will be ready to enter treatment, or engage in behavioral health care, the CHW will support them in identifying harm reduction goals to support the clients in behavior change. CHW will also provide community education to providers and other community partners to build capacity, help reduce SUD, HIV Stigma, and improve access to resources for clients in the community they work in. Essential Functions and Responsibilities Receive and respond to referral requests Conduct rapid testing, provide syringe exchange services and other prevention interventions Maintain up-to-date HIV, HCV knowledge; provide HIV/HCV education to clients and community partners Conduct searches for community resources and housing Assist the client in navigating through the health care system by providing information and advocacy and modeling appropriate communication Work with clients to address barriers to medical care, behavioral health and other services Assist clients in accessing pharmacy services needed to obtain medications Transport clients to and from community resources and assist them in accessing community resources Obtain Authorization for Release of Information between HIVA and Medical Provider when necessary Provide direct one-on-one and phone care services Facilitate client referrals to HIV Care Coordinators, Prime + Peers for assistance with HCV treatment, and PrEP navigators. Maintain up-to-date and accurate client records Participate in necessary meetings Assist with all necessary reports Participate in required training Comply with all applicable standards, guidelines, laws & administrative rules Other duties as defined by Director Required Qualifications TB test (provided at agency) Valid Oregon driver\'s license, or valid out of state driver\'s license with the ability to get an Oregon driver\'s license within 30 days of establishing residency in Oregon, and a driving record sufficient to be covered by agency auto insurance policy.
