Compensation (Pay): $21.43 to $31.07/hourly, based on experience.

A $1000 sign on bonus will be paid in the following manner: $300 payout upon 30 days of employment, $300 payout upon 90 days of employment, and $400 payout upon one year of employment*.

*Current employees are not eligible for the sign on bonus.

Loyalty Bonus - $1200 upon 1 year of service and then a $300 payout for every additional 3 months worked. This applies to only FT employees. The amount will be prorated for PT employees.

    Job Summary: The Collaborative Clinic at Academy is an integrated health clinic that focuses on the treatment and care for patients with HIV/Aids by focusing on primary medical and specialty needs. The CCA Medical Assistant functions as a liaison between the patient and members of the Care Team in a joint effort to optimize the patients’ health status and quality of life. The Medical Assistant will perform routine and emergent functions pertinent to efficient, ambulatory patient care.

    Essential Duties and Responsibilities

  • Provides patients with appropriate services and ensures coordination of these services through the nursing process, to include assessment, planning, facilitation, complex focused care coordination, evaluation, advocacy, and comprehensive care plan development to meet a patient’s complex needs.

  • Functions within interdisciplinary team to support team-based care including external partners from Southern Colorado Health Network (SCHN), Infectious Disease Specialists (IDS), and Colorado Department of Public Health and environment (CDPHE).

  • Supports the patient care providers in the performance of routine and acute clinical functions pertinent to efficient, ambulatory patient care. This would include all paperwork necessary for the appointment including and not limited to labs, reports from other agencies, scheduling appointments for medical needs, performing the necessary intake functions for arriving patients, assisting in questions, direction of telephone calls and other patient service duties.

  • Works with the CCA RN Coordinator to assure the schedules are reviewed and patients’ charts are prepared for the visit.

  • Identifies and links patients to available resources, social support systems, family support, and access to quality and cost-effective care.

  • Assesses high risk and rising risk patients consistent with the current Policies & Procedures for Care Management.

  • Provides education as needed to the patients, families, and caregivers to include disease processes, medication regimen, and navigation of barriers to care.

  • Encourages engagement and self-management of patients with regards to their own health.

  • Provides outreach to individuals as well as the community and in-reach to patients who are presently inactive in their healthcare.

  • Demonstrates an ability to utilize technology to include, but not be limited to, the Electronic Health Records, state registries, Health Information Exchange.

  • Maintains documentation of all interactions accurately and completely, implementing appropriate protocols for patients’ charts and billing practices.

  • Assists providers with inbox tasks to and completing pertinent paperwork for patients outside needs.

  • Understands the immunization schedule and is able support clinic staff and patients.

    Education/Experience:

    Education: Registered/Certified Medical Assistant & Current CPR (BLS) Certification.

    Work Experience: Relevant healthcare experience preferred. Experience with HIV/Aids patient population desired.

    Computer Skills: Basic – ability to access the intra/internet to manage timecard, review policies and procedures, and read company communications; use e-mail to communicate with co- workers, leadership, and other departments; enter and correct data, modify a workbook, format a worksheet, and use printing functions; create a simple presentation in PowerPoint, run it, and print it.
