Position Summary

Under the supervision of the VP of Youth Resiliency Services (YRS), the Navigation Coordinator will oversee the Youth Navigation Specialist Team, collaborating with Youth Housing Development Program (YHDP) partners to deliver trauma-informed assistance to young adults (16-24) facing housing instability or homelessness. Responsibilities include coordinating individualized support, conducting screenings, reviewing intake documents, and matching individuals with the appropriate Navigation Specialist. The role also involves supporting diversity, equity, and inclusion efforts through the application of anti-racist principles. Additionally, the Navigation Coordinator may handle a caseload and provide direct services as required. This position aligns with the Youth & Young Adult (YYA) driven initiative aimed at fostering innovative community responses to youth homelessness.

Some of the duties include:

  • Serve as point person for YHDP Navigation Team to coordinate activities, provide support, guidance, and direction to Navigation Specialists and liaison to the NAV Collaboration partners
  • Assist Navigation Specialists in the development of Navigation Plans, accessing resources, and making appropriate referral and linkage
  • Provide assessment and behavioral health services to transitional-aged youth and their families, creating solution-focused plans
  • Offer Medicaid-reimbursable services as needed, such as Psychological Diagnostic Evaluation, Community Psychiatric Supportive Treatment (CPST), and crisis response
  • Understand and implement a Positive Youth Development model while maintaining timely and accurate client records
  • Make referrals to advocate and liaise with schools, the justice system, and social and behavioral health services
  • Provide backup support to staff as needed and demonstrate cultural and clinical competence in the community
  • Stay current with required trainings, certifications, and licensure, ensuring compliance with licensing and accreditation standards
  • Maintain accurate documentation to comply with standards and requirements
  • Adhere to best practices including Trauma-Informed Care, Motivational Interviewing, Housing First, Harm Reduction, and Antiracism

Qualifications

Master's Degree in Social Work, Counseling, or related field required; with at least two years' experience working in community behavioral health working with Transition Aged Youth.  Experience in care coordination, case management, linkage and referral and system coordination on behalf of clients, and demonstrated experience with varied special populations and cultural competency.  Additionally, possessing a valid Ohio Driver's License with proof of insurance is a requirement for this position.
