Minimum Data Set Registered Nurse (RN) , Day Shift, Skilled Nursing Facility ( SNF) UC Health is hiring a part-time RN for SNF, for the first shift at Daniel Drake Center for Post-Acute Care. The MDS RN will support Daniel Drake's SNF department, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures. About Daniel Drake Center for Post-Acute Care Daniel Drake Center specializes in providing post-acute and transitional care services for patients recovering from a catastrophic injury or illness. Daniel Drake Center is the only hospital-based skilled nursing facility in the Cincinnati region. With a complete range of inpatient and outpatient services including medically complex care, skilled nursing, assisted living, wellness services, and research, Daniel Drake Center offers comprehensive care continuum and a variety of services to help patients throughout their recovery. Learn more at uchealth.com.The Minimum Data Set (MDS) Nurse coordinates and completes the Resident Assessment Instrument (RAI) and restorative plans of care in a timely and accurate manner. The MDS Nurse ensures that documentation is in compliance with federal long term care regulations, Prospective Payment System (PPS) requirements, and facility/departmental policies, procedure and rules. The MDS Nurse reviews documentation to monitor that the care plan is being followed and to ensure that staff is providing care that reflects the wishes of the resident. The MDS Nurse establishes restorative programs to enhance the resident's quality of life, improve or maintain functional status, and optimize case mix reimbursement as appropriate. The MDS nurse plays an active role in educating staff and interviewing residents according to MDS needs. The MDS nurse works closely with therapy and nursing team to maximize RUG scores.
