Crozer Keystone Health System Social Worker BSW Behavioral Health in Chester, Pennsylvania
Functions as an Administrative Case Manager to ensure mental health consumers have access to services that are necessary to tenure and progress in the community. Will assist any/all consumers requesting and eligible to receive services. Performs duties in support of the Medical Center's mission to ensure the highest quality of patient care in an economically sound and efficient manner. This position serves primarily adults, 18 and over, whose lives are affected by mental health problems.
Formulates accurate assessment statements relevant to data presented and consistent with current knowledge in the field, as evidenced by periodic record reviews and supervision sessions. Uses appropriate treatment interventions and available resources as measured by clinical supervision and review of progress notes.
Maintains charts according to regulations as measured by record reviews. Completes all authorizations, pre-certs, and reviews, both verbal and written, in order to obtain payment for services as demonstrated by the billing system and record reviews. Makes arrangements for appropriate referrals to other services and agencies under supervision, participates in outreach and case-finding activities, as evidenced by written reports
Ensures the productivity of the Case Management service by maintaining expected monthly utilization indicated by reports generated
Monitors clients who are referred to other facilities, completing follow-up as appropriate, noted in chart. Verified by review of records. Monitors and submits case management units of service as required, verified by billing reports and reviews of records. Meets with all case management clients on a regular basis as evidenced by documented contact. If face-to-face is not possible, all attempts to contact a client will be documented in the record.
Manages caseload as evidenced by caseload and record review. Understands confidentiality of records and consumer information. Complies with all areas of HIPPA.
A Bachelor’s degree in social work or a related field is required.
2 years or more experience in direct contact with mental health consumers.
Valid Motor Vehicle driver's license required.
