DEPARTMENT DESCRIPTION:

Cambridge Health Alliance is expanding services to address substance use disorders for its patients and communities. We are opening a low barrier and harm reduction Bridge Clinic at CHA Cambridge Hospital to provide treatment for patients who need immediate attention and who could benefit from additional clinical services. This program will be multidisciplinary and interdepartmental, and will collaborate with other departments throughout Cambridge Health Alliance and with our community partners in order to meet the needs of our patients with substance use disorders. This is a grant funded position.

Summary:

The BH Caseworker will primarily support the Bridge Clinic, by working directly with patients to assess and identify all psychosocial needs, help them and their families access vital services to address these needs, and consolidate available resources to facilitate these connections. By providing this support, the BH Case Worker should help improve both patients’ and providers’ experience of care, enable clinicians to spend more time delivering professional services, and connect patients to services that are vital for improving their overall health, functional status, and quality of life.

In alignment with the department’s Population Health efforts, the BH Case Worker will also support the clinic in various efforts to address top strategic priorities for improving the overall health of the patient population, addressing gaps in care for individual patients, and improving performance on key process and outcome measures designed to help CHA achieve care transformation, as incentivized by MassHealth. Reporting directly to the Operations Manager of Behavioral Health Case Work, the BH Case Worker will work closely with a wide range of staff, including operational leaders, clinical providers, quality managers, administrative staff, and clinical support providers.

Qualifications:

  • Education/Training: Bachelor’s degree in social work, psychology, public health, counseling, health education, or similar fields.

  • Work Experience: Minimum 2 years in a health care setting collaborating with clinicians and administrators; experience in behavioral health settings and direct interaction with service users, preferred.

  • Language: Ability to speak Spanish, Portuguese, or Haitian Creole strongly preferred.
