Resource TEam DOcumentation Specialist

JOB SUMMARY

The Resource Team Documentation Specialist serves as part of the Resource Team and assists Care Coordinators by reviewing various documents in the EMR for quality and compliance, helping with follow up as needed, and verifying that the identified issues were resolved in a timely manner.

ESSENTIAL FUNCTIONS

  • Participate in team meetings.

  • Contribute to the case review process as directed by the Resource Team policies and procedures.

  • Review documentation in EMR files of the people we support using checklists and reports to identify issues with quality and compliance that need correction or attention and opportunities for improvements.

  • Communicate with and review results of the documentation findings with leadership and Care Coordination team members.

  • Collaborate with the appropriate staff regarding issues found to make necessary corrections.

  • Forward any issues identified through the checklist or reports that might meet the necessary regulatory requirements for billing to the QA/Compliance department for evaluation and determination.

  • Identify opportunities for documentation improvement and share recommendations.

  • During the course of each review determine if any issues require escalation to supervisor and escalate them in a timely manner as needed.

  • Assist Supervisor in preparing summaries, trending and reports as needed.

  • Assist with follow-up and correction of documentation as needed.

  • Review records to confirm corrections were made as instructed by the Supervisor.

KNOWLEDGE, SKILLS & ABILITIES

  • Strong knowledge of OPWDD funded services and supports.

  • Knowledgeable of person-centered planning regulations.

  • Familiarity with Electronic Medical Record systems.

  • Ability to build relationships and effectively communicate.

  • Excellent customer service skills.

  • Strong organizational skills and attention to detail.

  • Excellent writing and word processing skills.

  • Ability to work autonomously.

  • Completes work in a timely manner and ability to prioritize.

  • Demonstrates cultural competence.

  • Ability to keep sensitive and privileged information confidential.

  • Demonstrates ethical and professional responsibilities and boundaries.

EDUCATION & EXPERIENCE

  • Bachelor’s degree required

  • Knowledge of OPWDD and/or DOH regulations, particularly related to Medicaid Service Coordination/OPWDD Care Coordination, required .

  • Care Coordination experience, required

  • At least 2 years of experience in the Quality Assurance and Corporate Compliance areas, preferred .

  • Experience conducting internal audit and monitoring activities in an OPWDD program/service, preferred
