RiseBoro Homecare Inc. Social Worker in BROOKLYN, New York
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Home Health Care Social Worker will participate in the development and promotion of the RiseBoro Homecares quality care initiative. The Social Worker is responsible for communicating with staff members, clients or caregivers regarding establishing homecare services for the client with MLTC/MCO, HRA, CHHA contracts and Private pay clients. Specific duties and responsibilities include the following:
Conduct mental health screenings to determine clients overall social emotional needs
Provide supportive counseling
Conduct in-home or virtual assessments and develop a plan of action to resolve the identified needs
Complete client contact surveys as required by contract, every six months; file appropriate documentation in clients' physical and electronic files
Complete spreadsheet/report for clients assigned and follow up with appropriate staff regarding information obtained in regards to concerns or complaints
Participate in the various meetings, as assigned i.e. Quality Improvement Committee, case conferences, staff trainings, etc.
Participate in case conferences as needed with the DPS, Field RN and Case Coordination for conflict resolution
Prepare survey report for Quality Improvement quarterly meetings
Screen for benefits and entitlements as appropriate
Work collaboratively with the Billing team to address any concerns as a result of the surplus report(s)
Assist in the converting of CHHA cases into MCO/MLTC cases
Participate in the education of HCWs by attending in service classes and facilitating discussions on Mental Health, i.e. Alzheimers/Dementia
Participates in outreach events as needed
Develop and implement eligibility reports/tracking system to document came management assistance provided to existing clients including information on:
documents necessary to maintain eligibility for home care services
calls initiated to contracts on client's behalf and
follow up with the Intake Department as well as other departments
Review the hold report list in system of record for both HRA and MCOs and conduct follow-up with clients and next kin for retention; advise appropriate staff regarding clients that need to be terminated
Review and addresses all messages on the HCAS report that relate to social/emotional needs or concerns and emails all appropriate staff
Perform other related duties as assigned by the Administrator
COMPETENCIES
Bilingual - Spanish preferred
SIFI certified or willingness to take the SIFI certification
Ability to organize, problem-solve and counsel regarding Home Health Care issues
Ability to recognize priorities in the organization of workflow
Able to perform duties independently with minimal need for direct supervision
Maintain confidentiality of all clients, other staff and the facility information
Demonstrate ability to interact in a positive and helpful manner with clients, visitors, co-workers and volunteers

Ability to work independently and collaborative as a team
Remains flexible in scheduling and staffing needs for the provision of optimal client care
EDUCATION EXPERIENCE
Master's Degree in Social Work required
