Overview
The Centralized Bereavement Coordinator position will directly report to the Centralized Bereavement Coordinator Manager and is responsible for participating with other members of the interdisciplinary team in five branches across North Arkansas in identifying the bereavement needs of the patient and family.

Essential Functions of Position
This is a safety-sensitive position.
Participates with the team in providing for the bereavement needs of families through, phone contact, in person visits,correspondence, and other types of personalized support.
Completes bereavement assessments per CoP's and the Organization Policy and Procedures
Maximizes family access to resources including volunteers, groups, and referrals to outside agencies.
Provides education and training to team members, and community and contract agencies as requested
by the supervisor and assists them in understanding the psychosocial dynamics associated with illness,loss, and death.
Maintains bereavement charts.
Provides monthly summary reports to include projects, phone calls, correspondence,
and cases closed.
Participates in the coordination of the patient's hospice care and in the hospice's quality assessment and
performance improvement program
Markets the company and bereavement services to the community and referral sources
Provides consultation and training to bereavement staff in other offices
Provides for the bereavement need of staff, including volunteers
Communicates with team members to provide and gather information to develop the family
bereavement care plan and to coordinate needed services
Performs other duties as assigned
About You
High School Diploma or equivalent required.
Undergraduate degree in human service field preferred
Counseling responsibilities require graduate degree in a related discipline
Minimum 1-year clinical experience in a varied multi-disciplinary setting; 3 years preferred Clinical experience in the areas of life-threatening and chronic illness, as well as grief and loss
Counseling experience in an individual, family, or group therapy preferred
Knowledge of terminally ill patients and their families and an understanding of end-of-life care, hospice, loss, and death.
