East Bay Community Action Program Nurse Care Manager in Barrington, Rhode Island

East Bay Community Action Program (EBCAP) is seeking a full-time Registered Nurse Care Manager for our Behavioral Health Services Division at our 2 Old County Road location in Barrington, RI. We are seeking highly professional and motivated individuals who enjoy working with a multidisciplinary team in a community health setting.

The successful candidate provides ongoing health assessments and care management to adults ensuring they receive assistance with patient advocacy, community health and educational services and referrals to community services and/or agencies. The successful candidate provides nursing and care management services to clients assigned to center prescribers. She/He builds relationships with internal and external medical providers to coordinate client's access to services including but not limited to medical appointments, specialists, hospitals, transportation, housing services and provides referrals to appropriate community services and agencies.

The successful candidate must have a minimum of an Associates Degree in Nursing or equivalent, and licensed as a Registered Nurse (RN) by the RI Department of Health. Prior experience in Psychiatric Nursing preferred. Previous experience in an electronic medical record (EMR) preferred; however, EBCAP provides training in EBCAPs EMR which is NextGen.
