This position is 40 hours per week, working Monday through Friday from 8:00am-4:30pm, hours are flexible and hybrid schedule.

Responsibilities

· Assesses appropriateness of referral to Kennedy Krieger by phone interview, medical records, or direct patient/family contact.

· Collaborates with the referral source, appropriate disciplines, and the family to plan the most effective evaluation or treatment program to meet proposed goals or outcomes.

· Collaborates with payor source, primary care provider, and Kennedy Krieger Institute Insurance Verification Specialists for financial approval of plans.

· Provides ongoing monitoring of the patient’s progress toward identified goals by reviewing treatment plans, physician recommendations, and/or progress notes. Utilizes a problem-solving approach to initiate and support changes needed to ensure quality, effective care.

· Participates in the multi-disciplinary team process to facilitate safe and appropriate discharge plans.

· Collects specific department utilization data, as well as patient outcome measures, for ease of access, quality, and cost effectiveness.

· Acts as an initiator and catalyst in the development of activities for innovative improvement of patient care outcomes.

· Participates in internal and external marketing/public relations activities including disseminating information in informal and formal education programs.

Qualifications

Education: Graduation from an accredited school of nursing. BSN or enrolled in a BSN program to be completed within 2 ½ years of hiring date.

Experience: Minimum 3 years of related experience in a hospital or home care setting required.

License: Active, unencumbered Registered Nurse license from Maryland or a compact state.

Certification: Current CPR certification that included hands on skills.

Related case management, quality assurance, or utilization review certification preferred.
