How you'll help us Keep Climbing (overview & key responsibilities)
A new role at Delta, the Manager – Care Coordination provides clinical leadership and program management expertise to lead a strategy and program to support Care Management for Delta employees.
Key responsibilities:
Drivesleadership and excellence in the care and experience of Delta employeesexperiencing a leave of absence from work relating to an on-the-job injury ordisability.
Oversees processes and system improvements to ensure high qualitycare management for employees across the Workers’ Comp and Leaves ecosystem atDelta.
Partners closely with multiple stakeholders, including Delta’sWellness Team (GHW), other Delta business units, the Workers’ Compensation andDisability Third Party Administrator, Delta Occupational Health clinic vendors,Nurse Case Managers and others, all while focusing on the employees’ health,wellbeing and recovery.
Leverages expertise in Disability and WorkersCompensation Case Management as well as leadership experience to documentprocesses, build standard operating procedures, identify gaps, and implementcontinuous improvements to build an integrated model for care management excellencefocused on the care and experience of injured and ill employees. itself.
Develops key clinical performance metrics to measure outcomes and ensure team’sobjectives are delivered to support corporate goals of coordinated, consistentexperiences for Delta employees.
Provides clinical expertise to consult oncomplex care reviews as needed. May be tasked with building a team ifapplicable andproviding leadership andoversight of direct report(s). Supports the Wellness team in creating anenvironment that improves employee health outcomes and the overall wellness ofour people.
This position is based in Delta General Offices in Atlanta, GA and reports into Delta’s Director of Health Services.
What you need to succeed (minimum qualifications)
Certified Nurse Case Manager
5 or more years of nurse case management experience with atleast 3 of those years in Workers’ Compensation and/or Disability/Leaves
3 or more years of leadership and managerialexperience
Broad clinical operations or nursing knowledge in theambulatory care setting
Proven experience managing outcomes and programs in aleadership capacity overseeing coordination between multiple departments orservices including, or similar to, employee health, occupational health and/orambulatory care settings.
Proven experience with documenting processes to identifygaps and recommend opportunities for improvement
Proven strong decision-making, problem-solving, projectmanagement and communication skills and demonstrate sound, independentjudgment.
Advanced ability to communicate to senior executives, frontline employees and large groups
Proficient in Microsoft Office applications and electronicmedical record software.
Consistentlyprioritizes safety and security of self, others, and personal data.
Embraces diversepeople, thinking, and styles.
Possesses a highschool diploma, GED, or high school equivalency.
Is at least 18 yearsof age and has authorization to work in the United States.
What will give you a competitive edge (preferred qualifications)
Experience working with Lean Management Systems.
Strong preference for candidates with a certification fromAmerican Academy of Ambulatory Care Nursing (AAACN). Strong preference forcandidates with current certification from the American Board for OccupationalHealth Nurses (ABOHN). Working knowledge of employee health, communicableillness management, infectious disease, travel health, and/or public health asit impacts a global employer.
