Description:

Position Purpose

The Case Management Coordinator is a leadership role responsible for overseeing non-clinical resource coordination for Mercy Care patients. This position ensures that individuals experiencing homelessness and other vulnerable populations have access to essential community resources, such as housing, employment, food, and transportation. Through direct supervision of community health workers and resource specialists, this role bridges the gap between social care and healthcare.

What You Will Do

  • Supervise and support a team of community health workers and resource specialists.

  • Oversee program administration, including budgeting, reporting, and documentation.

  • Develop and implement procedures to ensure compliance with policies, guidelines, and grant requirements.

  • Coordinate with clinic staff and social care teams to streamline resource access for patients.

  • Provide direct behavioral health case management, linking patients with necessary support services.

  • Train and onboard new team members to enhance program effectiveness.

Minimum Qualifications

  • Bachelor's degree in human services or a related field.

  • Three years of experience in social care programs, with expertise in connecting individuals to community resources.

  • Prior supervisory experience preferred.

  • Strong knowledge of community resources, including housing assistance, employment programs, and transportation services.

  • Ability to interpret policies and implement compliance measures.

  • Ability to identify best practices within scope of work and develop internal programmatic procedures and ability to coordinate with other internal partners for which there is no direct supervision.
