• Bachelors, Masters, or PhD in Nutrition or

• Registered Dietitian Nutritionist (RD/RDN)

Community Nutrition WIC Program

• Proficient use of NY/WIC Computer System

• Adhere to regulations of WIC Program according to NYS Policy.

• Interface with NYS department of Health representatives to ensure maximum compliance with WIC Program regulations.

Ensure confidentiality of all medical records.

Part time Day Shift

• Develop and implement high risk care plans and oversee high risk care to all participants.• Refer high risk participants to other health related and social services agencies as necessary.• Coordinate nutrition education, including breastfeeding promotion and support that is responsive to the identified needs/interest of each high risk participant.• Track high-risk participants progress in improving their health and document outcomes. • Provide leadership and assist in orienting, training and mentoring new local agency staff.• Conduct a complete nutrition assessment and determine program eligibility.• Conduct a complete income assessment to determine program eligibility.• Enroll participants.• Prescribe, tailor, authorize, issue and reissue food packages.• Provide and document any participant centered nutrition education (individual/family).• Provide and document follow up referrals to other agencies.• Coordinate participant scheduling.• Perform WIC Certifications and follow up visits.• Document test date and measurement results.• Provide telephone assistance to participants.• Provide leadership in planning, conducting, and evaluating Participant Centered Nutrition Education services.
