Palliative Care Coordinator Qualifications:
Must possess a current, unencumbered LPN License
Experience in scheduling in a healthcare organization or unit
Receive and promptly process palliative care referrals and/or inquiries for services provided by Crossroads
Work in cooperation with the call center for incoming or received referrals.
Adept knowledge of Palliative Care program and ability to explain to the fullest extent palliative care utilization and services available.
Maintain accurate and detailed records of all information calls and referrals.
Schedule and assign as appropriate all palliative patients with designated Palliative Care Nurse Practitioner(s).
Additional duties as assigned.
