Palliative Care Coordinator Qualifications:

  • Must possess a current, unencumbered LPN License

  • Experience in scheduling in a healthcare organization or unit

  • Receive and promptly process palliative care referrals and/or inquiries for services provided by Crossroads

  • Work in cooperation with the call center for incoming or received referrals.

  • Adept knowledge of Palliative Care program and ability to explain to the fullest extent palliative care utilization and services available.

  • Maintain accurate and detailed records of all information calls and referrals.

  • Schedule and assign as appropriate all palliative patients with designated Palliative Care Nurse Practitioner(s).

  • Additional duties as assigned.

Palliative Care Coordinator Schedule: Monday to Friday 8:30 AM to 5:00 PM
