Position: Behavioral Health Therapist - Licensed Clinical Social Worker
Job Status: Full Time; Monday - Friday
Salary: Salary Dependent on Experience

Essential Functions:
Promote and actively engage in behavioral health integration within primary care
Conducts therapy sessions with individuals or groups
Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical treatments and interventions
Provide quality psychosocial assessments of patients/clients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment and/or life situation
Ensure adequate programmatic data is collected, analyzed, and presented as necessary.
Assist in the development and distribution of Behavioral Health Program and Substance Abuse Program procedures/policies
Consultation with PCP's to enhance understanding of the patients/clients, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plan
Documents all patient/client related interactions and completes progress notes and discharge plans in a timely manner
Maintain confidentiality in compliance with HIPPA and RHC regulations
Requirements:
Master's Degree in Social Work with a minimum of two years' experience as a therapist and a current license to practice in Michigan as a Licensed Clinical Social Worker
State of Michigan Clinical Social Worker License
BLS required within 6 months of hire
